This aggregate report allows providers and coordinators to view information about non-compliant patients by VISN and facility. In this way the provider can identify non-compliant patients and determine what follow-up actions to take.
VISN: Select the VISN from the drop-down list. Multiple VISNs can be selected, as well as all VISNs.
Race: Select the race.
Start/End Dates: Click the calendar 
icons to select the start and end dates of the query, or enter the date in m/d/yyyy format.
The resulting report displays the results of the search in a grid format. The information includes the name and ID of each facility, as well as patient counts and percentages for compliance and non compliance
The data can be exported to different file formats, including Excel, comma-separated values (CSV), PDF, or Word.
The report can be sorted by column in ascending or descending order by clicking the corresponding column header.
A bar graph also displays the breakdown of compliance rates by facility.
Mammogram Follow-Up Compliance by Facility
The Mammogram Follow-Up Compliance page also allows users to view mammogram compliance follow-up information by facility.
Race: Select the race.
Start/End Dates: Click the calendar icons to select the start and end dates of the query, or enter the date in m/d/yyyy format.
Follow-Up Information: Select the type of patient follow-up action taken.
